The test looks at three things. First it reads the IP address and country that sites see. Then it asks your browser to resolve a few random names and records which DNS server those lookups come from. Finally it checks which addresses your browser reveals through WebRTC.
With the VPN on, all three should point to the VPN server. If your real address or your internet provider's DNS server shows up, part of your traffic is leaving outside the VPN.

No. The DNS test makes up a random ID; which DNS servers asked about it is kept in memory for ten minutes and then deleted. It is never linked to your account.
